4. Evaluation Plan
5. Evaluation Communication Plan
6. Sustainability Plan
7. One-Page Description of Coalition Outcomes (short-term, intermediate and/or long-term)

The application materials should be saved as ONE FILE (accepted formats are .pdf, .doc and .docx). Before submitting, do one final review of your application using the Phase I Grading Rubric!
