It is a\u00a0recommendation\u00a0to\u00a0Congress\u00a0with Congress having the ultimate power\u00a0and authority to authorize changes to Executive Branch agencies and to\u00a0appropriate federal\u00a0funds.\u00a0Proposals in the PBR\u00a0<\/span><strong>do not\u00a0represent\u00a0final numbers\u00a0appropriated\u00a0for federal programs. Congress must pass, and the President must sign, appropriations bills into law before any funding amounts can be considered final.\u00a0\u00a0<\/strong><\/p>\n<p><span data-contrast=\"auto\">The PBR\u00a0re-proposes\u00a0many of the same\u00a0substantial\u00a0cuts, program consolidations, and agency reorganizations from last\u00a0year.\u00a0Similar to\u00a0last year\u2019s PBR, many programs would either be eliminated or moved to\u00a0the\u00a0new Administration for\u00a0a Healthy\u00a0America (AHA), proposed as a new operating division in HHS.\u00a0<\/span><span data-ccp-props=\"{}\">\u00a0<\/span><\/p>\n<p><span data-contrast=\"auto\">According to the PBR, \u201c\u2026 [the] AHA Budget consolidates and streamlines chronic care and disease prevention programs across the Office of the Assistant Secretary for Health (OASH), Health Resources and Services Administration (HRSA), Substance Abuse and Mental Health Services Administration (SAMHSA), and several centers and programs of the Centers for Disease Control and Prevention (CDC) creating a more coordinated and efficient structure to serve to serve all Americans, particularly low-income, rural, and medically underserved individuals.\u201d\u202f\u00a0<\/span><span data-ccp-props=\"{}\">\u00a0<\/span><\/p>\n<p><span data-contrast=\"auto\">AHA would\u00a0impact\u00a0programs in the Office of National Drug Control Policy (ONDCP), Substance Abuse and Mental Health Services Administration (SAMHSA)\u00a0and the Centers for Disease Control and Prevention\u00a0(CDC).<\/span><span data-ccp-props=\"{}\">\u00a0<\/span><\/p>\n<p><span data-contrast=\"auto\">Details on the proposed cuts and\u00a0other\u00a0changes\u00a0in the FY 2027 PBR\u00a0to\u00a0the\u00a0substance use prevention infrastructure are below.<\/span><span data-ccp-props=\"{}\">\u00a0<\/span><\/p>\n<p><strong>ONDCP\u00a0<\/strong><\/p>\n<ul>\n<li><span data-contrast=\"auto\">The Drug-Free Communities (DFC) program\u00a0is proposed to\u00a0be moved from ONDCP to AHA and funded at\u00a0only\u00a0$70 million ($39 million less than the FY 2026\u00a0appropriated amount of $109 million).<\/span><span data-ccp-props=\"{}\">\u00a0<\/span>\n<ul>\n<li data-leveltext=\"o\" data-font=\"Courier New\" data-listid=\"2\" data-list-defn-props=\"{&quot;335552541&quot;:1,&quot;335559685&quot;:1440,&quot;335559991&quot;:360,&quot;469769226&quot;:&quot;Courier data-aria-level=\"2\"><span data-contrast=\"auto\">This proposed decrease in funding is a reduction of 36% from FY 2026.\u00a0This\u00a0could drastically scale\u00a0back\u00a0the national reach of the DFC program.<\/span><span data-ccp-props=\"{}\">\u00a0<\/span><\/li>\n<\/ul>\n<\/li>\n<li><span data-contrast=\"auto\">The Comprehensive Addiction and Recovery Act (CARA) Section 103 enhancement grant program\u2019s funding to current and former DFC grantees to do more with more intensity around opioid and stimulant issues is proposed to be<\/span> <strong>totally<\/strong> <strong>eliminated<\/strong><span data-contrast=\"auto\">\u00a0 (-$5.2 million).<\/span><span data-ccp-props=\"{}\">\u00a0<\/span><\/li>\n<li><span data-contrast=\"auto\">The High Intensity Drug Trafficking Area (HIDTA) program\u00a0is proposed to\u00a0be moved to the Department of Justice and funded at $196 million ($102.5 million less than the FY 2026\u00a0appropriated amount of $298.579\u00a0million).<\/span><span data-ccp-props=\"{}\">\u00a0<\/span><\/li>\n<\/ul>\n<p>&nbsp;<\/p>\n<p><strong>\u0421\u0410\u041c\u0421\u0410<\/strong><\/p>\n<p><span data-contrast=\"auto\">The PBR proposes to cut\u00a0a number of\u00a0SAMHSA\u2019s\u00a0Programs of Regional and National Significance (PRNS) across the three SAMHSA centers,\u00a0the Center for Substance Abuse Prevention (CSAP), Center for Substance Abuse Treatment (CSAT)\u00a0and the Center for Mental Health Services (CMHS).<\/span><span data-ccp-props=\"{}\">\u00a0<\/span><\/p>\n<p><strong><i>CSAP<\/i>\u00a0<\/strong><\/p>\n<p><span data-contrast=\"auto\">The PBR proposes to decimate CSAP.<\/span><span data-ccp-props=\"{}\">\u00a0<\/span><\/p>\n<ul>\n<li><span data-contrast=\"auto\">The following CSAP programs would be\u00a0<\/span><strong>eliminated<\/strong><span data-ccp-props=\"{}\">\u00a0<\/span>\n<ul>\n<li><span data-contrast=\"auto\">Enhancement Grants to current and former\u00a0DFCs to\u00a0do more with\u00a0more\u00a0intensity\u00a0around underage drinking (-$11 million)<\/span><span data-ccp-props=\"{}\">\u00a0<\/span><\/li>\n<li><span data-contrast=\"auto\">National Adult-Oriented Media Campaign (-$2.5 million)<\/span><span data-ccp-props=\"{}\">\u00a0<\/span><\/li>\n<li><span data-contrast=\"auto\">Interagency\u00a0Coordinating\u00a0Committee for the Prevention of Underage Drinking (ICCPUD) (-$1 million)<\/span><span data-ccp-props=\"{}\">\u00a0<\/span><\/li>\n<\/ul>\n<\/li>\n<li><span data-contrast=\"auto\">Tribal Behavioral Health Grants (-$22.665\u00a0million)<\/span><span data-ccp-props=\"{}\">\u00a0<\/span><\/li>\n<li><span data-contrast=\"auto\">Minority AIDS Initiative (-$43.2 million)<\/span><span data-ccp-props=\"{}\">\u00a0<\/span><\/li>\n<li><span data-contrast=\"auto\">Strategic Prevention Framework &#8211;\u00a0Prescription\u00a0Drugs (SPF-Rx) (-$10 million)<\/span><span data-ccp-props=\"{}\">\u00a0<\/span><\/li>\n<li><span data-contrast=\"auto\">Minority Fellowship Program\u00a0(-$1.3 million)<\/span><span data-ccp-props=\"{}\">\u00a0<\/span><\/li>\n<\/ul>\n<p>&nbsp;<\/p>\n<p><span data-contrast=\"auto\">The only programs previously\u00a0located\u00a0in CSAP and now proposed to be\u00a0funded and\u00a0moved to AHA are:<\/span><span data-ccp-props=\"{}\">\u00a0<\/span><\/p>\n<ul>\n<li><span data-contrast=\"auto\">The Prevention Technology Transfer Centers (PTTCs) funded at the FY 2026 final\u00a0appropriated\u00a0level of $9.5\u00a0million.<\/span><span data-ccp-props=\"{}\">\u00a0<\/span><\/li>\n<li data-leveltext=\"\uf0b7\" data-font=\"Symbol\" data-listid=\"1\" (PRNS)\u00a0are proposed to be funded at $490.202 million, a proposed cut of $74.017 million and would be moved to AHA.<\/span><span data-ccp-props=\"{}\">\u00a0<\/span><\/li>\n<li data-leveltext=\"\uf0b7\" data-font=\"Symbol\" data-listid=\"3\" from the Substance Use Prevention, Treatment and Recovery Services (SUPRTS) Block Grant (funded at\u00a0$2.008 billion\u00a0in FY 2026), the State Opioid Response (SOR) Grants (funded at\u00a0$1.595 billion\u00a0in FY 2026) and the Center for Mental Health Services (CMHS) Block Grant (funded at\u00a0$1.012 billion\u00a0in FY 2026).
