Registration for CADCA's 2005 National Leadership Forum will open in Fall 2004.

Friday, January 23, 7:00 am – 4:00 pm

Payment Policy
Registration forms are required for each attendee.

Member status with CADCA MUST be current in order to receive discounted rates. If you would like to verify your status, please contact CADCA member services at membership@cadca.org or 800-54-CADCA.

CADCA must receive full payment prior to attending the Forum. Any payments made on-site are subject to a $50.00 processing fee.

Registration Deadline
All registrations must be received by January 5, 2004. Registrations received after this will be considered an on-site registration and will be subject to a $50 service charge.

Scholarships
Scholarships covering the cost of registration and accommodations will be awarded to selected CADCA members in good standing thanks to the generosity of Forum sponsors and our Federal Partners.

Selection for scholarship awards will be based on financial need and participation in prevention or treatment programs. Coalitions that have not received previous scholarships will be given priority and will be awarded a maximum of one scholarship.

Purchase Order Policy
Purchase Orders are not considered payment. Registrations submitted with a Purchase Order will NOT be processed. Your registration form should be used as an invoice.

Registration Confirmation
Confirmations will be mailed to all attendees within 10 business days after receipt of registrations.

Cancellation Policy
There will be no refunds after December 31, 2003. Cancellations received in writing before this date will be refunded less a $25 administration fee. Walk-ins and replacements (in writing) are welcome.
